Job Summary
The Direct Care Staff is responsible for providing ongoing support, guidance, and supervision to clients in a residential setting. This role requires a strong commitment to ensuring a clean, safe, and home-like environment for clients.
Key Responsibilities
- Under the supervision of a qualified professional, provide support and guidance to clients to help them achieve their goals and improve their quality of life.
- Identify and address community integration issues for clients, including linking them with community resources and services.
- Develop and document comprehensive assessments and reviews, outlining rehabilitative goals and interventions that reflect client preferences and meet regulatory standards.
- Participate in the development of individualized action plans and attend treatment meetings, including weekly staff meetings.
- Document daily notes and communicate client progress, mental status, and any changes to relevant parties.
- Perform interventions with clients to address their identified goals and result in skill development, including crisis intervention and access to emergency services as needed.
- Provide linkages to other program staff and service providers to ensure a comprehensive support system for clients.
- Monitor and document medication administration, as required.
- Participate in core training and maintain certification of required trainings, including CPR/First Aid, Crisis Management, and MAP.
Qualifications
- Bachelor's Degree in a related field, or high school diploma/GED and one year of related experience.
- Excellent verbal and written communication skills, and ability to exercise sound judgment.
- Strong computer skills.
- Valid driver's license, access to an operational and insured vehicle, and willingness to use it to transport clients.
- Demonstrated understanding of and competence in serving culturally diverse populations.
