About the Role

We are seeking a highly skilled RN Case Manager to join our team at AccentCare, Inc. As a RN Case Manager, you will play a critical role in providing high-quality patient care and coordinating services to meet the unique needs of our patients.

Key Responsibilities
  • Conduct thorough assessments of patients' medical and social needs to develop effective care plans
  • Coordinate with healthcare providers, family members, and caregivers to ensure seamless care delivery
  • Monitor patients' progress and adjust care plans as needed to achieve optimal outcomes
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making
  • Collaborate with interdisciplinary teams to develop and implement care plans that meet patients' physical, emotional, and social needs
Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
  • One (1) year experience as a RN
  • Licensed to practice as a registered nurse in the state of agency operation
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
  • Ability to travel to all business locations
