About the Role

The Sub-Acute Nurse is a critical member of our multidisciplinary team, responsible for delivering high-quality nursing care to our clients. As a Sub-Acute Nurse, you will work closely with our clinical team to assess, plan, and implement individualized care plans that meet the unique needs of each client.

Key Responsibilities
  • Evaluate and assess client needs, identifying areas for improvement and developing strategies for care.
  • Collaborate with our multidisciplinary team to develop and implement individualized care plans.
  • Provide direct care to clients, including administering medications, performing treatments, and providing education and support.
  • Monitor client progress, making adjustments to care plans as needed.
  • Participate in quality improvement initiatives, identifying areas for improvement and developing strategies for enhancement.
  • Collaborate with our clinical team to ensure seamless transitions of care.
Requirements
  • Bachelor's degree in Nursing.
  • Active Registered Nurse license for the state in which you will be working.
  • Current CPR-BLS certification.
  • At least 21 years of age.
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements.
  • Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies' insurance policy.
  • Proof of up-to-date COVID-19 vaccination.
Preferred Qualifications
  • Psychiatric Nursing experience.
