Position Overview

Role Responsibilities:

  • Implement skilled physical therapy treatments as per physician directives while maintaining consistent communication with the medical team.
  • Document evaluations, progress notes, and discharge summaries for patients enrolled in the therapy program.
  • Collaborate with healthcare staff to address the unique needs of individuals, promoting effective rehabilitation and functional maintenance.
  • Participate in team meetings and provide insights during the absence of the Director of Physical Therapy.
  • Maintain accurate statistical records of patient treatments.
  • Assist in training programs for staff as needed.
  • Update and manage individual care plans within the Inter-disciplinary Kardex.
  • Request necessary supplies for treatment from the Director of Physical Therapy.
  • Assume responsibilities of the Director of Physical Therapy during their absence, including oversight of Physical Therapy Aides and ensuring all documentation is completed.

Qualifications:

  • Graduation from an accredited physical therapy program is required.
  • A valid state license to practice physical therapy is mandatory.
  • Preferred experience of two years in the field of physical therapy.
  • Ability to lead and direct others is essential.
  • This position necessitates regular interaction with residents, colleagues, and supervisors, requiring proficiency in reading, writing, speaking, and understanding the English language at an intermediate level or higher.
