About Our Organization:
Range Mental Health Center is dedicated to serving the community as a Certified Community Behavioral Health Clinic (CCBHC).

The mission of the CCBHC initiative is to enhance access to mental health and substance use disorder services, particularly for underserved populations, promote the integration of behavioral and physical health care, and ensure the consistent application of evidence-based practices.


Position Overview:


This position encompasses both direct patient care and administrative responsibilities, tailored to address the needs of our community in alignment with our CCBHC designation.

The Practical Nursing Specialist will deliver care throughout the organization, with a predominant focus on specific areas of service.

Key programs involved in this role include substance use disorder treatment (detoxification and inpatient services) and outpatient psychiatric care.


Core Responsibilities:
Practical Nursing Specialists (PNS) collaborate closely with supervising nurses and healthcare providers to evaluate and monitor patient needs, ensuring that care and treatment are executed in accordance with established protocols by:
- Providing compassionate care to patients
- Documenting patient medical histories, symptoms, and vital signs with accuracy and professionalism.
- Updating patient records and electronic health documentation (EHR).

- Performing clinical tasks such as blood draws, administering injections, and responding to urgent medical situations, including first aid and wound care.

- Administering medications and implementing treatment plans as recommended by the healthcare team.
- Adhering to nursing regulations, standards, and organizational policies.
- Collaborating with colleagues, other departments, and external agencies.
- Completing continuing education requirements and participating in performance evaluations as required.
- Upholding the organization's code of conduct, maintaining appropriate communication and boundaries with staff, patients, and external entities.

Additional duties may be assigned within the scope of practice for PNS, addressing the needs of the organization and its patients.


Program-Specific Duties:


- Conducting chart audits, reviewing policies, and updating procedures to ensure compliance with regulatory standards and safe care delivery.

- Reporting findings to nursing staff, clinical managers, program directors, and other relevant stakeholders.

- Providing training and support to unlicensed assistive personnel regarding medication administration, documentation, health assessments, and emergency protocols.

- Actively participating in program meetings and offering recommendations for enhancing patient care and organizational services.
- Collaborating with various departments to ensure timely and safe service delivery to patients.

- Engaging with external agencies, including hospitals and social services, to facilitate efficient referrals and care coordination while adhering to privacy regulations.

- Flexibility to assist in different programs/services based on staffing requirements and patient acuity.

Required Skills and Qualifications:
- Strong clinical competencies complemented by excellent verbal and written communication skills.
- Ability to delegate tasks effectively and openness to receiving guidance and feedback.
- Exceptional prioritization and time management abilities, with a proactive approach to problem-solving.
- Demonstrated ability to collaborate under challenging circumstances, maintaining professionalism in all interactions.
- Proficiency in Microsoft Office Suite and experience with electronic health records (EHR).

While efforts will be made to limit work outside standard hours, our organization operates 24/7/365 to meet patient needs.

Willingness to work various shifts, including evenings, nights, and weekends, is essential to fulfill patient and organizational requirements.


Education and Experience:
- Current licensure as a Practical Nurse in Minnesota.
- New graduates are encouraged to apply.
- Previous experience in behavioral health, community health, or substance use services is beneficial.
- Successful completion of a State of MN Department of Human Services criminal background check is required.
- CPR Certification is mandatory.
- Valid driver's license with a clean driving record

Physical Requirements:


- Ability to sit for extended periods, perform standing and bending tasks, and lift up to 50 pounds as needed.

- Must be capable of responding to medical emergencies.
- Minimum age of 18 years.
- Ability to perform medical procedures, including injections.

- Sufficient vision to operate a computer, monitor patients, and accurately dispense medications.

- Adequate hearing to communicate effectively in person and over the phone, both individually and in groups.
- Clear articulation of the English language for effective communication.
- Maintain a professional appearance and adhere to cleanliness standards.
