SUMMARY:
As a vital member of our healthcare team, the Pharmacy Technician specializing in medication history plays a crucial role in ensuring the safety and efficacy of patient care. This position is responsible for gathering and documenting comprehensive medication histories for patients, which is essential for optimal treatment outcomes.

KEY RESPONSIBILITIES:
1. Accurately obtain and record medication histories from patients upon admission.
2. Collaborate with healthcare providers to ensure accurate medication reconciliation.
3. Support the Manager of Clinical Pharmacy Services in delivering high-quality pharmacy services.

QUALIFICATIONS:
- A formal education equivalent to an Associate's degree (2 years of college) is required.
- A minimum of 3-5 years of relevant experience in a pharmacy setting.
- Certification from the Pharmacy Technician Certification Board (PTCB) is mandatory.
- Registration as a Pharmacy Technician with the Maryland Board of Pharmacy is required.
