We are seeking a skilled and compassionate Care Plan Nurse LPN to join our team at St. James Place Retirement Community. As a key member of our healthcare team, you will play a vital role in ensuring the timely and accurate completion of Minimum Data Set (MDS) assessments, care plans, and other supporting documentation.
Responsibilities- Conduct thorough and accurate MDS assessments, including Resident Assessment Protocols (RAPs) as needed, in accordance with federal, state, and local regulations.
- Develop and implement individualized care plans that meet the unique needs and goals of each resident.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Provide direct resident care as assigned, including assistance with daily living activities and medical procedures.
- Maintain accurate and up-to-date documentation, including MDS, nursing notes, care plans, and therapy notes.
- Current Registered Nurse licensure or Licensed Practical Nurse (LPN) certification.
- Minimum Data Set (MDS) training and experience.
- Progressive nursing experience, preferably in resident assessment.
- 3-5 years of long-term care experience.
- Proficient computer skills, including Microsoft Office applications and other software relevant to the position.
