Job Summary:

We are seeking a highly skilled Nurse Practitioner to join our dynamic post-acute Cardiology, Nephrology, and Pulmonology practice. As a key member of our team, you will work closely with supervising Physicians to provide exceptional care to sub-acute and long-term patients in the Mesquite, Tx area, Ft. worth, Tx area, and the north Dallas region.

Responsibilities:
• Provide high-quality patient care in a post-acute setting
• Collaborate with Physicians and other healthcare professionals to develop and implement patient care plans
• Conduct thorough assessments, diagnose conditions, and develop treatment plans
• Educate patients and their families on disease management and self-care
• Stay up-to-date with the latest medical research and best practices in Cardiology, Nephrology, and Pulmonology

Requirements:
• Certified Nurse Practitioner (CNP) or equivalent
• Current licensure in the state of Texas
• Strong clinical skills and experience in Cardiology, Nephrology, and Pulmonology
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
