View Point Health's Centerville Play Therapy & Resilience Training Center provides culturally, linguistically, and developmentally appropriate education and practice of resilience skills through play and family therapy, focusing on children from infancy to age 17 and their families. The Engagement & Care Coordinator plays a crucial role in delivering direct care to enhance program and service engagement by identifying needs related to social determinants of health, coordinating care for children and their families, and ensuring effective referral, linkage, monitoring, and follow-up. This role is performed in a dynamic environment that requires strong organizational skills, professionalism, problem-solving abilities, and a commitment to community service.

Key Responsibilities:
  • Facilitates direct care coordination to improve client and family engagement in services. Conducts initial assessments of client needs and assists in connecting them to necessary resources.
  • Handles and screens telephone and email inquiries professionally and courteously, providing clear and accurate program information.
  • Oversees engagement activities, including communication with external partners, supports appointment scheduling, and conducts in-person, telehealth, and community visits with clients and families, while monitoring attendance reports.
  • Utilizes social determinants of health and other assessment tools to identify client and family needs. Collects program-specific information using electronic health records to document activities and advocacy efforts.
  • Establishes and maintains connections with external partners and community organizations, manages referral processes, and tracks linkage to care, including discharges.
  • Assists in arranging primary care and other health appointments, therapy, and enhances access to additional resources that promote integrated care and family wellness.
  • Collaborates with an interdisciplinary team to reduce behavioral health disparities and enhance overall engagement in care.
  • Connects clients with community services and provides follow-up and monitoring to ensure continuity of care among internal and external providers.
  • Facilitates collaboration to develop person-centered or family-centered integrated client treatment plans.
  • Interacts with team members to provide comprehensive and timely assistance in resolving barriers to service access.
  • Documents and maintains accurate and timely case notes.
  • Assists clients in making and coordinating appointments and obtaining necessary supports to keep appointments.
  • Facilitates communication between providers within established timeframes.
  • Develops and maintains strong working relationships with community service providers.
Minimum Qualifications:

Bachelor's degree in a Social Services related field from an accredited institution AND one year of experience in a Social Services related role OR a high school diploma or GED AND three years of experience in a Social Services related position OR one year of experience at the lower level Social Services Technician 2 (SST011) or an equivalent position.

Preferred Qualifications:
  • Experience with CareLogic or other EHR/EMR systems.
  • At least two years of experience in behavioral health care or a child and family serving organization.
  • Bilingual – ability to read, write, and speak in both Spanish and English.
Requirements/Competencies:
  • Ability to lift 20 pounds.
  • Requires long periods of sitting and standing.
  • Must be at least 21 years of age.
  • Must possess a valid driver's license and maintain a Motor Vehicle Record in accordance with company policy.
  • Candidates must pass a criminal background check (including fingerprinting).
  • Must pass a pre-employment drug screen and be subject to random drug screenings.
  • Any combination of training and experience that would enable the applicant to acquire the necessary knowledge, skills, and abilities. Note: Some positions may require a valid driver's license.
  • Other duties as assigned.
