Job Description

This is a PRN opportunity as a Home Health Nurse with our team in Dallas. The estimated salary for this position is $65,000-$90,000 per year, depending on experience.

  • Provide skilled nursing care to patients in their homes
  • Assess and monitor patient conditions, recognizing changes that may require intervention and implementing care to prevent or reduce risk
  • Collaborate with interdisciplinary healthcare teams to enhance continuity of care
  • Administer medications and treatments as prescribed
  • Educate patients and their families on healthcare plans directed toward improving independence and functionality
Requirements

To be considered for this role, you must:

  • Be a licensed LPN/LVN in the state of employment
  • Have at least 1 year of experience as an LPN/LVN
  • Have Medicare home health experience (preferred)
  • Have a basic understanding of OASIS (preferred)
  • Have Homecare Homebase HCHB experience (preferred)
  • Demonstrate strong communication and interpersonal skills
  • Have a valid state driver's license, automobile liability insurance, dependable transportation, and be able to safely drive in all types of weather
  • Hold CPR certification for the healthcare provider
