**Job Overview**

We are seeking a Licensed Practical Nurse (LPN) to join our team at Children's Healthcare of Atlanta. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the hospital's mission to make kids better today and healthier tomorrow.

**About the Role**

This is a full-time position working 40 hours per week, Monday through Friday. You will work a variety of shifts, including 8-hour days from 8:00 AM to 6:30 PM and 10-hour shifts from 10:00 AM to 8:30 PM, with one rotating day off.

As an LPN, you will work under the supervision of a provider or registered nurse, participating in the assessment, planning, implementation, and evaluation of care. You will perform the nursing process while providing direct patient care in a safe and effective manner that promotes quality patient outcomes.

**Responsibilities**

Assists licensed professional personnel in performing the nursing process across the continuum to deliver age and developmentally appropriate patient-family-centered care.

Utilizes clinical decision support tools and resources to recognize issues and prioritize care.

Provides patient and family education appropriate for identified learning needs.

Communicates with home health agencies, third-party payors, and other community resources as needed to coordinate needs.

Facilitates and provides ongoing communication with patient/family, interdisciplinary staff, and third parties (home health, third-party payers, pharmacies, and other resources) to identify and resolve potential barriers.

Performs outpatient and clinic care coordination and monitors patients' care between clinic and home.

Facilitates care via telehealth to assist with patient needs and concerns.

Participates in providing discharge instructions as delegated by licensed professional practitioners.

**Requirements**

No minimum experience required; preferred qualifications include 1 year of patient care experience.

Education requirements include a high school diploma or equivalent and completion of an accredited Licensed Practical Nurse (LPN) school/program.

Certifications include Licensed Practical Nurse (LPN) and Basic Life Support (BLS) within 30 days of employment.

PALS or PEARS Certification within one year of hire date, as required by department specification.

**Salary Information**

The estimated salary for this position is $65,000-$85,000 per year, depending on experience.
