We are seeking a skilled Physical Therapist to join our Home Health team at E-talentnetwork. As a Physical Therapist, you will play a vital role in providing high-quality patient care and developing effective treatment plans to improve patient outcomes.
Key Responsibilities- Evaluate patients and develop personalized treatment plans to address their physical therapy needs.
- Conduct thorough assessments and evaluations to determine patient diagnoses and develop plans of care.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care and support.
- Supervise Physical Therapy Assistants and Home Health Aides to ensure high-quality patient care.
- Participate in training and support of team members, volunteers, and students.
- Prepare clinical and progress notes to document patient care and outcomes.
- Associate/Bachelor's Degree in Physical Therapy (Required)
- 1 year of Skilled Home Health Experience
- Knowledge of growth and development principles throughout the lifespan
- Demonstrated ability to provide age-appropriate care
- License Physical Therapy (Pending license also acceptable)
- CPR/BLS Certification
