Position Overview:
Rochester Manor is seeking a dedicated and skilled MDS Coordinator to join our nursing team. This full-time role is essential in aligning with our organizational goals while ensuring optimal reimbursement for patient care services.

Key Responsibilities:
As an MDS Coordinator, you will:

  • Engage in the Patient Assessment Process by completing the Minimum Data Set (MDS) and the corresponding Care Area Assessments (CAAs).
  • Assist in formulating individualized patient care plans, identifying patient needs, and projecting outcomes in compliance with Federal and State regulations.
  • Contribute to the interdisciplinary Resident Assessment Instrument (RAI) process, adhering to RAI guidelines for MDS completion, computer encoding, and electronic submission.
  • Provide training on Activities of Daily Living for new hires and facilitate ongoing education for existing staff.

Qualifications:
Candidates must be graduates of an accredited School of Nursing with a current Registered Nurse (RN) license. A minimum of one year of experience in long-term care nursing is required.

$93,600 - $104,000 / yearly
