Hospice LPN Job Description

ViaQuest Hospice is seeking a compassionate and skilled LPN to join our team. As a Hospice LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Responsibilities:
  • Assist RNs or Physicians in performing specialized procedures, duties, and plan of care items.
  • Administer and instruct administration of medication.
  • Maintain current patient records and progress notes.
  • Maintain regular communication with RN concerning patient and family care needs.
Requirements:
  • Graduate of an accredited nursing program.
  • LPN license.
  • Nursing or patient care experience preferably within Hospice specialty.
  • Valid driver's license, car insurance, and reliable transportation.
  • Willingness to travel throughout assigned service area.
