- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with the healthcare team to ensure comprehensive care
- Participate in interdisciplinary rounds and meetings to discuss patient care
- Master's degree in Social Work (MSW) or related field
- Licensed Clinical Social Worker (LCSW) certification
- Minimum 2 years of experience in palliative care, hospice, or related field
