Job Title: Provider Relations Representative

Location: Within a 20-mile radius

Job Type: Full-Time (40 hours per week)

Working Hours: Monday to Friday, 8:00 AM - 5:00 PM

Position Overview: We are seeking a motivated and independent Provider Relations Representative to facilitate access to medical records and ensure quality measures are met.

Key Responsibilities:



  2. Travel to various healthcare facilities within a designated territory.

  3. Pull and review patient charts, focusing on three main measures: diabetes management (A1C levels), blood pressure readings, and depression screening.

  4. Conduct straightforward assessments, including verifying if recommended actions were taken based on patient data.

  5. Collect data from electronic medical records (EMRs) as well as paper charts.

  6. Collaborate with the Provider Relations team to establish introductions with practice managers and explain the necessary access to records.

  7. Build and maintain relationships with various practices and their staff.

  8. Effectively communicate and coordinate with internal teams and external stakeholders.

  9. Work independently while managing a flexible schedule.



Qualifications:



  2. Experience in medical records, as a medical assistant, or in a front office role is preferred.

  3. Must possess the ability to manage your own schedule and work autonomously.

  4. Strong interpersonal and communication skills.

  5. Proficiency in Mandarin or Cantonese is a significant plus, but not required.
