  • Work collaboratively with a clinical care team to deliver comprehensive care to patients in their homes
  • Perform thorough health assessments, diagnose, and treat complex illnesses
  • Provide therapeutic interventions, such as splinting, suturing, wound care, and minor procedures
  • Use critical thinking skills and evidence-based standards of practice to ensure quality care
  • Accurately document patient encounters and ensure continuity of care
  • Educate patients and families on managing their health and well-being
  • Communicate effectively with patients, families, and healthcare teams
Requirements:
  • Minimum of two years of experience as an Advanced Practice Provider in Emergency Department, Urgent Care, Internal Medicine, or relevant areas
  • Graduate with an advanced degree from an accredited institution
  • Current unrestricted state license as a Nurse Practitioner or Physician Assistant
  • Nationally board certified, current
  • Current BLS required, ACLS certification preferred
  • Prescriptive authority and DEA
  • Valid driver's license with clean driving record
  • Ability to lift and carry equipment up to 50 pounds
  • Ability to work flexible schedules, including evenings, weekends, and holidays
