Job Summary

12 Oaks Management is seeking a highly skilled Medication Aide to join our team. As a Medication Aide, you will play a vital role in ensuring the health and well-being of our residents by administering medication in a safe and efficient manner.

Key Responsibilities

  • Administer medication to residents in accordance with their care plans and under the direction of licensed nurses.
  • Document and chart medication administration, as well as report any observations or changes in resident condition.
  • Assist residents with activities of daily living, such as hygiene, toileting, feeding, and mobility.
  • Perform assigned treatments and provide incidental household services as needed.
  • Maintain confidentiality of resident information and adhere to HIPAA policies and procedures.
  • Collaborate with the care team to ensure seamless delivery of care services.
  • Participate in ongoing training and education to enhance skills and knowledge.

Requirements

  • High school diploma or equivalent required.
  • At least six months of experience in an assisted living community preferred.
  • Certified as a Nursing Assistant (CNA) and Registered Medication Aide in the state of employment.
  • Ability to lift up to 75 pounds and stand for long periods.
  • Strong organizational and problem-solving skills.
  • Ability to work independently and as part of a team.
