Home Health Speech Therapist, Comprehensive Care Specialist

We are seeking a skilled Home Health Speech Therapist to join our comprehensive care team. As a key member of our multidisciplinary team, you will work closely with patients and families to provide personalized speech therapy services in their homes.
Job SummaryThe primary responsibility of this role is to evaluate, direct, and provide speech/language pathology services to patients in their homes or facilities. You will participate in the development and periodic review of the Plan of Treatment and Plan of Care, utilizing your professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing, and swallowing.
Key Responsibilities:
- Evaluate patients' communication and swallowing abilities, identifying areas for improvement and developing effective treatment plans.
- Direct and implement speech therapy interventions, using evidence-based practices and adapting to individual patient needs.
- Collaborate with interdisciplinary teams, including nurses, physical therapists, occupational therapists, and social workers, to ensure comprehensive care coordination.
- Maintain accurate and timely documentation of patient progress, outcomes, and treatment plans, as per Company policy and regulatory requirements.
- Communicate effectively with patients, families, caregivers, and healthcare providers to ensure seamless care transitions and successful patient outcomes.

Benefits and CompensationWe offer a competitive salary range of $76,800 - $105,800 per year, based on experience and qualifications. Additionally, our benefits package includes paid time off, 401(k) retirement savings plan, employee assistance program, business travel insurance, and more.
RequirementsTo be considered for this role, you must meet the following requirements:
- Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
- A minimum of six months of experience as a speech therapist/speech language pathologist.
- Home health experience is highly desirable.
- A current and unrestricted license to practice speech-language pathology.
- Current CPR certification.
- Good organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation.
