Job Title: Home Health Physical Therapist

LHC Group is seeking a highly skilled and dedicated Home Health Physical Therapist to join our team in Newark, New Jersey.

About the Role:

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. They will work closely with the physician to develop a therapy plan of care and provide interventions aimed at improving and enhancing the patient's well-being.

Responsibilities:
  • Evaluate patient's functional status, status of all body systems, and physical therapy needs.
  • Consult with physician in development of therapy plan of care.
  • Provide services within scope of practice as defined by state laws governing the practice of physical therapy.
  • Observe and report patient's response to treatment and any changes in condition to nurse supervisor and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
  • Instruct and inform participating members of healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals.
  • Visit patients within entire service area of respective agency and other nearby LHC agencies as needed.
  • Complete progress notes for each visit and submit per LHC policy.
  • Complete all patient evaluations and develop PT plan of care within state-specific guidelines.
  • Report outcomes of evaluation, goals, and anticipated projected frequency of care to nurse supervisor within 24 hours of evaluation.
  • Participate in staff conferences and committees as requested by agency.
  • May supervise home health aides per LHC policy if allowed per state regulation.
  • Provides supervision of PTA as per LHC policy and state regulation.
  • Schedule flexibility is needed in this role to ensure we provide care to patient as needed.
  • Requires ability to participate in weekend rotation and potential availability on observed holidays to ensure we are meeting demands of our patients.
  • Prepare written discharge summary and/or written summary report on patient's condition at least every 60 days.
  • Provide in-service education as requested by agency leadership.
  • Adhere to LHC Therapy Standards of Practice in provision of services to all patients.
  • Responsible for accessing education to maintain and enhance clinical knowledge and skills that are up to date with current standards of care for profession and to maintain state-issued licensure.
Requirements:
  • Current Physical Therapy licensure in state of New Jersey.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
