Job Summary

As a Pharmacist at CAMC Health System, you will provide clinical and distributive pharmacy services to patients of all ages. Your expertise will be essential in preparing medications, assessing and monitoring medication therapy, and providing drug information to nursing and medical staff.

Responsibilities

• Provide accurate and timely drug information to practitioners and professional staff.
• Communicate pharmaceutical information clearly, professionally, and concisely.
• Consult with practitioners and professional staff as needed to clarify or ensure positive therapeutic outcomes and minimize drug-related problems/errors.
• Label and dispense drugs accurately and completely, and in a timely manner.
• Review patient profiles for any interactions based on diagnosis, allergies, or duplications via order entry and respond appropriately to electronic alerts.
• Ensure dosing is appropriate for patient's weight and age.
• Perform pharmacokinetic monitoring according to protocol, including ordering and interpretation of labs and ordering of drug regimens.
• Monitor medication therapies identified in the pharmacy department policy.

Safety and Quality

• Educate patients and family members as needed, using age, cultural, and psychosocial needs, and living conditions of the patient and/or family as a guide.
• Participate in quality improvement and medication use review activities of the department.
• Conduct effective literature evaluations.
• Demonstrate appropriate sterile aseptic technique, order of mixing, use of compounding equipment, and other pharmaceutical preparation practices.
• Ensure controlled substances and corresponding records are handled appropriately.
• Verify accuracy of all products prepared for patients by technicians.

Leadership

• Supervise the duties of pharmacy technicians performed during shift.
• Direct and assist pharmacy technicians in the completion of tasks.
• Demonstrate the ability to make critical decisions in a timely manner.
• Adapt positively to frequent interruptions and changes in workload and/or work schedule.
• Assist in the orientation of new pharmacy employees and pharmacy students for experiential rotations.
• Assist with special projects as requested.

Financial

• Sustain and assist in management of the formulary.
• Demonstrate concise, accurate, and complete documentation for all patient-specific records, following established standards of practice.

Knowledge, Skills & Abilities

• Medium Work: occasional lifting and/or carrying of objects weighing up to 25 lbs with a maximum lifting of 50 lbs.
• Training in proficiency of age-specific therapies for ages served by the pharmacy.

• Bachelor's Degree (Required)
• Doctorate Degree (Required)
