We are seeking a highly skilled and dedicated MDS Coordinator to join our team at Providence Care Center. As a key member of our leadership team, you will play a vital role in fostering a culture of compassion, inclusion, integrity, excellence, and collaboration.
Key Responsibilities:- Accurate Resident Assessments: Conduct and coordinate the development and completion of resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
- Timely MDS Completion: Ensure accurate and timely completion of the MDS process to ensure seamless care delivery.
- Interdisciplinary Collaboration: Collaborate with the interdisciplinary team to develop individualized resident care plans that meet their unique needs.
- Progress Monitoring: Monitor and document resident progress and responses to care plans to inform future care decisions.
- Quality Assurance: Participate in the Quality Assurance and Performance Improvement (QAPI) process to drive continuous quality improvement.
- Effective Communication: Communicate with residents, families, and other healthcare providers to coordinate optimal care delivery.
- Regulatory Compliance: Stay updated on MDS regulations and guidelines to ensure compliance and maintain the highest standards of care.

- Excellent Communication Skills: Excellent organizational, communication, and interpersonal skills are essential.
