We are seeking a highly skilled and dedicated MDS Coordinator to join our team at Providence Care Center. As a key member of our leadership team, you will play a vital role in fostering a culture of compassion, inclusion, integrity, excellence, and collaboration.

Key Responsibilities:
  • Accurate Resident Assessments: Conduct and coordinate the development and completion of resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
  • Timely MDS Completion: Ensure accurate and timely completion of the MDS process to ensure seamless care delivery.
  • Interdisciplinary Collaboration: Collaborate with the interdisciplinary team to develop individualized resident care plans that meet their unique needs.
  • Progress Monitoring: Monitor and document resident progress and responses to care plans to inform future care decisions.
  • Quality Assurance: Participate in the Quality Assurance and Performance Improvement (QAPI) process to drive continuous quality improvement.
  • Effective Communication: Communicate with residents, families, and other healthcare providers to coordinate optimal care delivery.
  • Regulatory Compliance: Stay updated on MDS regulations and guidelines to ensure compliance and maintain the highest standards of care.

Requirements:
  • Current RN License: A valid Registered Nurse (RN) license is required.
  • RAC-CT Certification: RAC-CT (Resident Assessment Coordinator-Certified) certification is mandatory.
  • MDS Expertise: Strong understanding of MDS 3.0, RAI process, and care planning.
  • Excellent Communication Skills: Excellent organizational, communication, and interpersonal skills are essential.
  • Team Player: Ability to work independently and collaboratively within a team.
