Job Description:

POSITION SUMMARY:

The Hospice Nurse at HexaQuEST Health, Inc. is responsible for admitting patients to service, assessing their physical, psychosocial, environmental, safety, and developmental needs.

They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment.

This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program, and conducts follow-up visits as needed.


Territory Details:
Oakland, Berkeley, Alameda, San Leandro, San Lorenzo, Castro Valley, Hayward, Union City, Fremont, and neighboring cities as needed.

Requirements:
One year of Hospice experience.

Position Urgency:

Normal Shifts:
Days: 8:30am-5pm (Schedule can be flexible based on patient's availability.)

Weekends:
One weekend per month.

State License Details:
Must Be Currently Active Minimum Years of Experience: 1 Minimum Guaranteed Hours: 40
