Primary Responsibilities:

  • Conduct in-home assessments, including past medical history review, system evaluation, medication reconciliation, vital signs, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing (as necessary)
  • Identify diagnoses to inform care management and active medical management
  • Formulate lists of current and past medical conditions using clinical knowledge and judgment
  • Communicate findings to inform members' primary care physicians of potential gaps in care
  • Recognize urgent and emergent situations and intervene appropriately
  • Educate members on topics such as disease processes, medications, and compliance
