Job Summary

We are seeking a skilled LPN Clinic Facilitator to join our team at Affinia Healthcare Inc. As a key member of our healthcare team, you will provide safe and efficient care and services to patients, working under the general supervision of the Assistant VP of Health Center Operations.

Key Responsibilities
  • Assist in planning and implementation of patient-based care based on sound nursing judgment, clinical preparation, knowledge, skills, and experience.
  • Coordinate clinic services, requiring strong verbal and written communication skills.
  • Triaging walk-ins (emergency patients) as appropriate, and possessing the ability to coordinate the activities of a busy provider practice.
  • Monitor and track follow-up of diagnostic test results, under the direction of the provider, and provide related education, counseling, and ongoing review.
  • Perform specialized procedures, examinations, and/or tests as ordered, and collaborate with physicians to provide extensive health education, counseling, and advice to patients and families.
Requirements
  • Graduate of an accredited school of practical nursing.
  • Requires (4) four years' experience working in a community health setting, with experience coordinating clinic services.
  • Strong verbal and written communication skills, computer literacy, and the ability to coordinate the activities of a busy provider practice.
  • LPN license from the State of Missouri, CPR certification, and a valid driver's license with insurance coverage.
