About the Role:

SSM Health is seeking a skilled and compassionate Nurse Practitioner to join our team at St. Mary's Care Center. As a key member of our healthcare team, you will provide high-quality nursing care to our residents, ensuring their physical, emotional, and spiritual needs are met.

Responsibilities:

  • Provide skilled nursing care to residents, administering medications and treatments according to established care plans and federal and state regulations.
  • Collaborate with the healthcare team to develop and implement individualized care plans, ensuring residents receive comprehensive and coordinated care.
  • Conduct regular assessments, identifying and addressing potential health risks and concerns.
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care transitions and address any concerns or questions.
  • Participate in report meetings, sharing knowledge and expertise to enhance the quality of care provided.
  • Ensure accurate inventory of medications, monitoring for potential side effects and ensuring proper distribution.
  • Develop and implement preventative measures to promote resident comfort, safety, and well-being.
  • Respond promptly to alarms, ensuring timely interventions to prevent potential harm.
  • Collaborate with the healthcare team to provide high-quality end-of-life care, ensuring residents and families receive compassionate and dignified support.

Requirements:

  • Graduate of an accredited school of nursing.
  • Basic Life Support HealthCare Provider (BLS HCP) certification.
  • Registered Nurse (RN) licensure in the state of work location or compact multi-state license.
