The Hospice of Santa Cruz County is seeking a skilled and compassionate Nurse Case Manager to join our interdisciplinary team. As a key member of our team, you will be responsible for providing high-quality patient care and case management services to patients and their families in our hospice program.
Key Responsibilities- Develop and implement individualized plans of care that reflect patient and family goals and preferences.
- Collaborate with the interdisciplinary team to coordinate care and services, including medication management, symptom control, and bereavement support.
- Provide education and support to patients and families on end-of-life care, pain management, and other relevant topics.
- Document all patient interactions, care plans, and outcomes in accordance with agency policies and procedures.
- Participate in quality improvement activities to ensure high-quality patient care and services.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Current California RN license.
- 2-5 years of RN experience in hospice, oncology, med-surg, ICU, ER, skilled nursing facilities, or home care.
- 1 year of acute care experience preferred.
- Previous experience working with electronic medical records is desirable.
- Hospice and/or home health experience preferred.
- End-of-life and pain management experience preferred.
- Previous experience working with end-of-life care.
- Bilingual in English/Spanish.
This position requires the ability to work in a variety of settings, including private homes and care facilities. The successful candidate will have strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
