PACT Vocational Specialist

Eliot is seeking a skilled professional to fill the role of a Masters Level Vocational Case Manager for the Program for Assertive Community Treatment (PACT). As a key member of our team, you will work closely with consumers to develop treatment interventions that support their recovery process.

• Act as a primary case manager to 7-15 consumers, providing individualized vocational support to assist them in identifying their strengths, barriers to work, establishing goals, and managing symptoms and work.
• Participate in treatment planning meetings, providing written documentation and clinical discussions to ensure effective collaboration with consumers.
• Take the lead role in providing vocational services to consumers, developing sound and working relationships with local employers, career centers, employment advocates, and DMH to assist consumers in finding employment.
• Provide job coaching to individuals employed competitively in the community, accompanying them to the workplace, observing their performance, and offering assistance and support as needed.
• Interface with client supervisors to ensure that individuals are completing designated tasks as assigned.

Monday - Friday 8am to 4pm, with one 12pm-8pm shift per week and one weekend per month. Rotating on-call duties and rotating holiday schedule.
