- Ensure follow-up with appropriate resources and services, promoting a smooth and effective care pathway.
- Collaborate closely with providers as the primary patient liaison, supporting the physician-patient relationship and fostering a culture of compassion and care.
- Contribute to data collection, analysis, and identification of areas for improvement, driving continuous quality enhancement.
- 2-3 years' experience in a healthcare setting, with a strong background in nursing and patient care.
- Preferred experience in spine and pain care, with a focus on patient navigation and advocacy.
- Current RN license in the state of practice, with a strong commitment to delivering high-quality patient care.
- Preferred certifications include BLS and ACLS, with a focus on pain management and patient safety.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
