We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at Physicians Services Group. As a key member of our post-acute care team, you will work closely with our attending physicians to provide high-quality patient care to skilled nursing and assisted living facilities, as well as rehabilitation patients.
Key Responsibilities- Deliver Patient-Centered Care: Provide direct patient care based on treatment plans set forth by hospital discharge summaries and attending physicians.
- Collaborate with Healthcare Team: Work closely with primary care physicians, specialists, and other healthcare professionals to coordinate patient care and ensure seamless transitions.
- Assess and Treat Patients: Evaluate and treat new admissions, follow-ups, and scheduled routine Medicare follow-ups based on PSG guidelines.
- Document Patient Information: Maintain accurate and up-to-date patient records within our electronic medical record system.
- Communicate with Families: Keep family members informed about patient status and progress.
- Comply with Regulatory Requirements: Ensure compliance with relevant laws, regulations, and facility policies.
- Certification and Licensure: ANCC certification or equivalent, SC license preferred, and registered nurse certification.
- Experience and Skills: Strong clinical background and experience in hospital or acute settings, with excellent interpersonal and communication skills.
- Education and Training: Advanced medical knowledge of common chronic disease states and ability to recognize and solve acute problems.
- Competitive Compensation: $90,000 - $120,000 per year.
