Oncology Nurse Navigator, Part Time

Baptist Health is seeking a highly skilled and compassionate Oncology Nurse Navigator to join our team. The successful candidate will be responsible for coordinating clinical consultation, patient education, treatment, and plan of care coordination for all patients. This role requires strong communication and collaboration skills, as well as the ability to work independently and make decisions autonomously.
Key Responsibilities:
Facilitate high-quality and individualized patient care through coordination of treatment plans and collaboration with providers.
Serve as a liaison between the organization, patients, families, and the community for oncology care at Baptist Health Paducah.
Develop and maintain necessary databases and outcomes registries to support research, improve clinical procedures, track patient outcomes, and enhance processes for efficient and compliant patient management.
Demonstrate initiative, accountability, and autonomy in developing the Nurse Navigator program needs and goals for success.
Requirements:
Current RN licensure recognized by the Kentucky State Board of Nursing is required.
BSN degree is required.
Three years of job-related experience is required.
BLS and ACLS certifications are required.
