In this role, you will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each resident. You will also be responsible for communicating effectively with residents, families, and other healthcare professionals to ensure seamless care coordination.

Responsibilities
  • Provide day-to-day care for residents, including assistance with ADL's and responding to resident requests
  • Utilize PointClickCare (PCC) Healthcare Software to chart and access patient data
  • Collaborate with the interdisciplinary team to develop and implement personalized care plans
