RN Case Manager

South Shore Health is seeking a skilled RN Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring the highest quality of service.
Job Summary:The RN Case Manager will work closely with patients, families, and healthcare providers to develop and implement individualized care plans. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities:- Coordinate patient care and develop individualized care plans
- Collaborate with healthcare providers and patients to ensure seamless transitions of care
- Conduct thorough assessments of patient needs and develop plans to address those needs
- Communicate effectively with patients, families, and healthcare providers to ensure understanding and compliance with care plans
- Monitor patient progress and make adjustments to care plans as needed
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Registered Nurse (RN) license in the state of Massachusetts
- 3-5 years of experience in acute care or case management
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of healthcare financing, community resources, and patient care processes
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification
- Experience with managed care and post-acute care community resources
