Key Responsibilities:

  • Participate in the development and implementation of individualized care plans that meet the unique needs of each patient.
  • Provide skilled nursing services, including wound care, medication management, and education to patients and their families.
  • Collaborate with the interdisciplinary care team to ensure seamless coordination of care and achieve optimal patient outcomes.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
  • Communicate effectively with patients, families, and healthcare team members to ensure that all needs are met and that care is delivered in a patient-centered manner.

Requirements:

  • Current RN licensure in the state where the branch is located.
  • At least one year of clinical nursing experience in an acute care or long-term setting.
  • Home health experience preferred but not required.
  • Current knowledge of fundamental medical-surgical nursing principles and practice.
  • Excellent clinical nursing skills and ability to work independently.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to use a computer and have a working knowledge of a variety of computer applications.
  • CPR certification.
