About the Role

We are seeking a skilled Speech Language Pathologist to join our team at Revival Home Health & Hospice. As a Speech Language Pathologist, you will be responsible for evaluating and treating patients with speech, language, cognitive, and swallowing disorders.

Responsibilities
  • Evaluate and treat patients with speech, language, cognitive, and swallowing disorders based on physician orders/referrals
  • Complete comprehensive evaluations using appropriate evaluation and assessment tools and procedures
  • Review patient records for current diagnosis, past medical history, precautions, and contraindications
  • Establish measurable goals and develop treatment plans to move patients toward their maximum level of speech, language, cognitive, and swallowing abilities
  • Regularly reassess effectiveness of treatment plans, progress of patients toward goals, and the need for modifying goals or treatment
  • Communicate patient needs and progress to the treatment team, physician, person receiving services, family members, as applicable
  • Consult with medical providers regarding patient evaluation and treatment, progress of patients, and discharge from treatment
  • Maintain and submit evaluation, treatment, and billing documentation in the format and time-lines required by the Company, facility, funding source, local, state, and federal regulations
Qualifications
  • Master's Degree in Speech Language Pathology
  • Current Speech Language Pathology licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
  • One (1) year of experience in speech pathology, home health experience preferred
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills; speech language pathology skills per competency checklist
