St. Paul's PACE program is dedicated to providing comprehensive care for individuals aged 55 and older who are facing chronic health challenges or disabilities. Our mission is to enable these individuals to maintain their independence while receiving the necessary medical support in their homes and communities.
Role Responsibilities:
As a Certified Nursing Assistant (CNA), you will work under the guidance of the In-Home Services CNA Supervisor. Your primary responsibilities will include:
- Assisting participants with daily living activities and providing restorative and supportive care.
- Monitoring health status and ensuring a safe environment for all participants.
- Collaborating with an interdisciplinary team to deliver high-quality care.
Essential Qualifications:
- Licenses and Certifications: Current California CNA certification is required. Successful completion of First Aid and CPR training is also necessary.
- Education: A high school diploma or equivalent is mandatory. Completion of a formal CNA training program is required for CNAs.
- Experience: A minimum of one year of experience working with the elderly or frail population is essential.
Key Duties:
In your role, you will:
- Consult daily with the In-Home Services CNA Supervisor regarding participant care plans and scheduled visits.
- Assist participants with personal hygiene, mobility, meal preparation, and medication management.
