Key Responsibilities- Make home visits to new patients in designated geographic territories, tailoring care plans to meet their unique needs.
- Perform assigned duties, including medication administration, wound care, treatments, and procedures, always prioritizing patient safety and comfort.
- Monitor patient conditions and report any changes to our Clinical or Client Services Manager, ensuring seamless coordination of care.
- Accurately document observations, interventions, and evaluations, utilizing a state-of-the-art touch pad tablet to maintain detailed records of patient care management and services provided.
- A current license as a Registered Nurse in Vermont and New Hampshire.
- A minimum of one year of recent, verifiable clinical experience.
- Prior home care experience is strongly preferred but not required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- The ability to work independently and manage time effectively.
- Strong interpersonal skills, with the capacity to build trust and rapport with patients and their families.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
