The ideal candidate will have a strong background in coronary care and experience working in a cardiac rehab unit. You will be responsible for assessing patient health problems and resources, developing and implementing individualized care plans, and providing education to patients and families.

Responsibilities
• Assess patient health problems and resources, taking a leadership role in the development, implementation, and outcomes evaluation of individualized care plans.
• Provide professional nursing care and interventions to cardiac adult patient populations, including those in inpatient and outpatient settings.
• Collaborate with the multidisciplinary team to deliver comprehensive care and support to patients and families.
• Educate patients and families on disease management, self-care, and lifestyle modifications to promote optimal health outcomes.
• Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote patient-centered care.
Qualifications
• Minimum one (1) year coronary care experience.
• Minimum two (2) years current experience working in a cardiac rehab unit preferred.
• ADN, Diploma, BSN, or Foreign Equivalent.
• Current AZ State RN License in good standing.
• Current BLS card.
• Current ACLS card within 6 months of hire.
