Job Summary

The Licensed Practical Nurse (LPN) is responsible for providing high-quality, patient-centered care to individuals in their homes. This role requires a strong commitment to delivering compassionate and personalized care, while working collaboratively with the healthcare team to achieve optimal patient outcomes.

Key Responsibilities
  • Provide direct nursing care to patients in their homes, as prescribed by the physician.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
  • Administer medications, perform wound care, and provide other treatments as ordered by the physician.
  • Document patient visits thoroughly and accurately, using electronic medical records and other tools as required.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • Current LPN licensure in the state of practice.
  • Current CPR certification.
  • Previous home care experience preferred.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
