Pearls Coach

We are seeking a skilled Pearls Coach to join our team at Circle of Care St. Louis. The successful candidate will provide late-life depression coaching services through an evidence-based program, PEARLS.

• Respond to referrals to determine needs, eligibility, and appropriateness for PEARLS and other Circle of Care, St. Louis programs.
• Deliver the PEARLS intervention to older adults and caregivers experiencing depression, working in clients' homes or virtually.
• Coordinate services, maintain contact with care receivers, and family members as needed.
• Report back to referral sources and communicate with clients' primary care providers regarding progress and outcomes.
• Complete record keeping in Circle of Care, St. Louis database(s).
• Participate in monthly consultations with the agency's social services team.
• Initiate and maintain collaborative relationships with community agencies and organizations.
• Present information to the community regarding depression, PEARLS, and other Circle of Care, St. Louis services.
• Attend meetings, staff development activities, trainings, and special events as directed.

The ideal candidate will have an Associate degree in social work, counseling, human services, nursing, or a related field, with experience working with older adults and mental health. Strong written and verbal communication skills are essential, as well as the ability to work in clients' homes and use technology to deliver services.

The successful candidate will work in a combination of office and home settings, with occasional driving required for appointments outside of the office. The position requires flexibility in scheduling and carrying out priorities, as well as the ability to handle and resolve stress.
