Key Responsibilities

  • Provide education and support to patients and their families to promote mental health and well-being
  • Develop and implement discharge plans to ensure continuity of care and optimal outcomes
  • Document patient information accurately and timely in accordance with clinical guidelines and standards
Requirements
  • Master's degree in Social Work or related field from an accredited institution
  • Current Utah State licensure as LCSW, CMHC, or LMFT
  • Ability to obtain additional licensure in states where tele-crisis services are contracted
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced, dynamic environment
