Job Title: Registered Nurse - Home Health Admission

BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to join our team as a Home Health Admission professional. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and admission services to clients in their homes.

Job Description:

As a Home Health Admission Nurse, you will be responsible for:

  • Conducting thorough assessments of clients' needs and developing personalized care plans
  • Providing education and support to clients and their families on health management and self-care
  • Collaborating with healthcare teams to ensure seamless care transitions and coordination
  • Documenting client information and progress in a timely and accurate manner
Requirements:
  • A current license as a Registered Nurse in NJ
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience is strongly preferred, but not required
  • Graduation from an accredited and approved nursing program
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills
  • Solid computer skills; prior experience with electronic medical records (EMR) is preferred
