As a Community Health Worker, you will conduct program assessments using program and CareStar identified tools and process. The Position- Bachelor's Degree
- 12 months experience in home and community-based services within the past 5 years
- Knowledge of Medicare, Medicaid, home health care and Home and Community-Based waiver programs desirable
- Adheres to the CareStar Rule in performance of job responsibilities
- Understands and complies with CareStar Policies and Procedures
- Maintains confidentiality as related to patient information
