Nurse for Home Health Patients

This is a unique opportunity to join Aveanna Healthcare's team of dedicated nurses, who are passionate about delivering exceptional care to patients in their homes. As a Private Duty Nurse, you will work closely with patients and their families to develop personalized care plans that address their specific needs and goals.
Key Responsibilities:
* Developing and implementing individualized care plans for patients
* Providing high-quality, patient-centered care that addresses physical, emotional, and social needs
* Collaborating with other healthcare professionals to ensure seamless care coordination
* Maintaining accurate and timely documentation of care provided
* Educating patients and their families on health promotion and disease management
Requirements:
* Active, unencumbered nursing license (RN/LPN/LVN) in the state of practice
* Current CPR certification from American Red Cross or American Heart Association
* TB skin test (current within last 12 months)
* Six months prior hands-on nursing experience preferred
