We are seeking a skilled and compassionate Physical Therapist to join our team as an In-Home Outpatient (IHOP) Physical Therapist at RVNAhealth.

About the Role

This is a full-time position that will cover Central/Southern Fairfield County. As an IHOP Physical Therapist, you will play a vital role in providing outpatient physical therapy services to a diverse patient population including post surgical total joint replacements. Your responsibilities will include:

  • Comprehensive evaluation of patient's level of function and participation in development of the total patient care plan with, patient, family, and physician.
  • Identification of patient and family needs for other home health services and referral for same when needed.
  • Treatment of patients per protocols and requirements of PT practice act per State of Connecticut.
  • Participation in case conferences as needed.
  • Communication with Wellness Center, patient and family as needed while working within ALFs.
  • Instruction of patient, family and other agency health care personnel in the patient's treatment regime when indicated.
  • Assists patients to reach a level of function to enable them to remain safely in their home environment.
  • Obtains written physician's order for Physical Therapy every sixty (60) days, more often if necessary.
  • Performs safety evaluation prior to the patients total joint replacement and subsequent visit following surgery.
Requirements

To be successful in this role, you must have:

  • Graduate with a baccalaureate degree of an accredited school of Physical Therapy approved by the American Medical Association Council of Medical Education with collaboration with the American Physical Therapy Association.
  • Current license to practice as a Physical Therapist in Connecticut.
  • At least two (2) years' clinical experience.
  • Knowledge of physical therapy methods with skill in casework in the part B home setting.
  • Knowledge and understanding of the principles of Part B home care and billing.

The estimated salary for this position is around $80,000-$100,000 depending on experience, which includes competitive compensation and extensive benefits.
