About the Job:

As a Nurse Care Specialist at Summit BHC, you will be responsible for providing high-quality nursing care to patients in a behavioral health setting. Your primary goal will be to assess, plan, and implement individualized care plans that address the unique needs of each client.

Key Responsibilities:

  • Assess clients' physical and behavioral health needs and develop comprehensive care plans.
  • Provide education and support to clients and their families on various health conditions and recovery strategies.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Monitor clients' progress and adjust care plans as needed.
  • Document all client interactions and care activities in a timely and accurate manner.


Requirements:

  • Graduation from an accredited school of nursing.
  • Valid current RN license from the state or a compact state.
  • One or more years of nursing experience in a behavioral health setting preferred.
