Job Summary

We are seeking a compassionate and skilled Pediatric Home Health Nurse to join our team. As a Pediatric Home Health Nurse, you will provide one-on-one care to our pediatric patients, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Provide high-quality, patient-centered care to pediatric patients in their homes
  • Develop and implement individualized care plans in collaboration with our interdisciplinary team
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient progress and adjust care plans as needed
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Requirements
  • RN/LPN with a current valid license to practice as issued by the State Board of Nursing
  • Current CPR certification
