Care Planning Specialist

**Job Summary:**

We are seeking a highly skilled Care Planning Specialist to join our team at Village At Cook Springs. As a Care Planning Specialist, you will play a critical role in ensuring the timely and accurate completion of the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), and Triggers. Your expertise will be instrumental in developing and maintaining policies and procedures related to data gathering and care planning.

**Key Responsibilities:**

* Develop and coordinate a tracking system for Quality Assurance related to the MDS, RAPs, and Triggers
* Conduct and coordinate the revision of the MDS, RAPs, and Triggers
* Maintain an up-to-date and accurate MDS/assessment calendar
* Ensure MDS compliance with timelines, accuracy, and signature of data
* Provide orientation and in-service to staff, family, and resident, and physicians, as necessary, to ensure understanding of the MDS/care planning process
* Maintain and implement an organized, efficient system to ensure submission of all MDS/care plan information to the State timely and accurately
* Participate in care plan meetings and ensure notification of all team members, family, and resident

**Requirements:**

* Degree in nursing, social work, or dietetics required
* Minimum of two years of clinical experience, preferably in a Long Term Care setting
* Knowledgeable of the MDS process

**Physical Requirements:**

* Must be able to move about intermittently throughout the workday
* Must be able to cope with the mental and emotional stress of the position
* Must be able to reach, bend, and/or stoop intermittently throughout the workday
* Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met
* Must be able to push, pull, or move a minimum of 100 pounds utilizing proper body mechanics and assistance
* Must be able to walk/stand 75% of the day

**Work Environment:**

* Must be able to work in a fast-paced environment with multiple priorities and deadlines
* Must be able to work effectively in a team environment
