  • Job Summary

    The Nurse Practitioner (NP) will provide direct patient care to Arizona Priority Care members to improve outcomes in quality, patient experience, and help mitigate utilization of resources in support of our organizational goals and objectives.

    This position requires autonomy with a focus on disease prevention and health management. The successful candidate will be working in the East Valley.

  • Key Responsibilities

    The NP shall:

    • Provide quality patient care within NP scope of practice and ensure delivery of effective and appropriate care consistent with established regulatory, licensing, and evidence-based practice standards.
    • Perform patient annual wellness visits in various settings, including private residences, assisted living facilities, primary care provider offices, specialist offices, hospitals, or skilled nursing facilities.
    • Conduct chart reviews to assist in supporting proper diagnoses, status, and plan of care specific to patient acute and chronic conditions.
    • Assess patients for all acute and chronic conditions and diagnose each condition that impacts the patient's care and treatment.
    • Adhere to documentation standards and complete documentation of assessed acute and chronic conditions, including correct diagnosis code, status of condition, and provider plan to address each diagnosis code.
    • Demonstrate knowledge and proficiency in understanding key elements of Risk Adjustment Factor (RAF) and Hierarchical Condition Categories (HCC) model measures and how these relate to our organizational goals and objectives within 90 days of hire.
    • Demonstrate knowledge and proficiency in understanding key elements of Quality STAR (HEDIS) measures and how these relate to our organizational goals and objectives within 90 days of hire.
    • Educate and counsel patients/families on preventive services and ordered recommended screening tests applicable, such as DRE, A1c, DEXA, Mammogram, and Colorectal screening.
    • Demonstrate knowledge and proficiency in understanding co-existing disease condition interactions and properly diagnosing and assigning diagnosis codes.
    • Educate and guide patients on disease prevention and health promotion throughout their lifespan, with particular focus on the aging process and geriatric population health.
    • Promote a safe environment, adhering to Arizona law mandates regarding abuse, neglect, or exploitation reporting.
    • Notify patients/designees of any recommended changes to the plan of care.
    • Prescribe, order, administer, and dispense therapeutic measures, including pharmacologic agents and devices, in accordance with Arizona State Board of Nursing Advance Practitioner rules and regulations.
    • Makes therapeutic adjustments and/or recommendations to medications as needed.
    • Orders, performs, and interprets diagnostic tests and screening evaluations, such as lab work, diagnostic imaging, spirometry, ankle/brachial index (ABI), and point-of-care testing, providing timely follow-up communication to patients of results.
    • Work collaboratively and cooperatively with AZPC organization members and other agencies to coordinate care and services required by patients.
    • Maintain knowledge and proficiency in medical practices through continuing medical education (CME).
    • Participate in development of the AZPC Home Wellness Program to meet the healthcare needs of AZPC members.
    • Participate in and contribute to HWP staff orientation, education, and development upon request.
    • Participate in the interview and selection process of HWP staff upon request.
    • Function as an internal resource for clinical and non-clinical team members to promote positive patient/customer outcomes.
    • Other duties as directed.
