Nurse Liaison

We are seeking an experienced RN Infusion Liaison to join our dynamic team at Providence Home and Community Care. As a key member of our infusion services team, you will provide high-quality nursing care to patients in various settings, prioritizing education, empowerment, and collaboration.
**Job Summary:**
- Deliver infusion nursing care to patients in hospital, home, clinic, or alternate living sites
- Develop and implement personalized care plans, ensuring seamless transitions and optimal outcomes
- Work collaboratively with multidisciplinary teams to coordinate care and address patient concerns
Responsibilities:
* Provide direct nursing services related to infusion therapy* Collaborate with patients, caregivers, and healthcare providers to ensure comprehensive care planning* Participate in multidisciplinary teams to coordinate care and address patient concerns* Develop and implement personalized care plans to promote optimal outcomes* Conduct thorough assessments and identify potential barriers to care* Implement interventions to overcome identified barriers and promote successful transitions* Educate patients and caregivers on infusion therapy techniques, medication management, and self-care strategies* Maintain accurate and timely documentation of patient information and care activities* Participate in quality improvement initiatives to enhance infusion services* Engage in ongoing education and professional development to stay current with best practices and evidence-based guidelines* Collaborate with interdisciplinary teams to develop and implement policies and procedures to ensure compliance with regulatory requirements and organizational standards* Represent Providence Home and Community Care in community outreach and partnership efforts to promote awareness of infusion services and foster collaborative relationships with community stakeholders* Participate in performance improvement activities to enhance patient satisfaction, reduce readmissions, and improve overall quality of care* Develop and maintain relationships with community partners, payers, and healthcare providers to ensure access to infusion services and optimize patient outcomes
